Medical Records Request

To request your medical records, please download and complete the form below. We kindly ask that you complete the form using a computer or print legibly. If you need assistance, our staff is available to help.

Once completed, please return the form to us via one of the following methods:

Processing Time:
Please allow up to 10 business days for your request to be processed.

Once your request is ready, we will send the documents to you via an encrypted file using a secure platform called Cisco. Please note, you will need to sign up for a free account to access this file.

• Be sure to check your spam/junk folder, and add medrecs@rdhosp.org to your Safe Sender list to ensure communication from our department reaches you.

For large files, a USB drive may be used as the preferred method of delivery. All flash drives are encrypted, and passwords will be mailed separately for security.
